Form\/document purpose:
<\/strong>Applicant and product identification, applicant declaration, establishment of nominated person for the relationship with PCD, official notification to RAR regarding the intention to certify the products.<\/p>\n

<\/strong>\u2013 Point 2.1: CERTIFICATION SYSTEM and CERTIFICATION SCHEME \u2013 see CHAP. 6 OF RG-00<\/a><\/p>\n

<\/strong>Establishing the documents to be submitted to the file.<\/p>\n

Additional information:
<\/strong>\u2013 The certification scheme chosen when completing the certification application is used;<\/p>\n

\u2013 On the column corresponding to the number of the certification scheme, the documents to be submitted to the file are identified.<\/p>\n

<\/strong>Self-assessment of technical capability and quality management requirements for certification.<\/p>\n

Additional information:
<\/strong>\u2013 Complete by hand and tick the options applicable to the company;<\/p>\n

– The annex with the organization chart of the company must be attached to the form.<\/p>\n

Requires registration: <\/strong>NO<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n

The forms are printed on the printer, filled in and then sent by fax<\/strong> to no. +4-021-350 82 88 or they are scanned and sent by e-mail<\/strong> to the address<\/span> dcp@rarom.ro<\/a>, together with the other documents specified in the List of documents requested for authorization.<\/span><\/span><\/p>\n

The completed forms can also be brought in person<\/strong> to the PCD offices.<\/span><\/p>\n
